By fulfilling the role of a primary care physician, NOVA House Call Physicians coordinates patient care with other health care providers. We work diligently to decrease unnecessary visits to the emergency room and to the hospital.

With regular physician home visits, medication management, and care coordination, we work to improve the health of homebound and geriatric patients.

We work with assisted living and senior living facilities.

Many of the residents at assisted living and independent living facilities need a wide variety of physician services. At NOVA House Call Physicians, we provide these services to patients inside the facility.

By utilizing our quality senior care services, patients and residents will be happier and require fewer unnecessary hospital and emergency room visits. Our goal is to work proactively with facility management care team to prevent emergency situations and provide quality care.
