Whom do We Serve?

We are a Medicare provider, and accept most Medicare Advantage and private insurance plans in addition to traditional Medicare. If you regularly require in-home health care services or have physical or mental conditions which make it difficult for you to leave your home you may qualify for a NOVA physician house call services. We serve home-bound seniors in assisted living, independent living, retirement communities, senior apartments, and private homes.

Our In-Home Medical Services include, but not limited to:

• Comprehensive review of your medical history and Medications
• In-home physical examinations and ongoing treatment of chronic medical conditions
• Medication management and medication refills
• Referrals to  in-Home physical therapy and specialized In-Home rehabilitation services
• Coordination of all home care services and medical equipment
• Community placement consultations
• Physician review for authorizations (Home health authorization, Medical equipment authorizations, Diagnostic testing authorizations)

• NOVA House Call Physicians fulfills the role of a primary care physician as our In-Home medical care providers are highly trained in the diagnosis and treatment of homebound, critically complex and geriatric patients.  With regular physician home visits, medication management, and care coordination, we work to improve the health of homebound and geriatric patients.Our In-Home medical care providers are equipped to perform most of the same services that you would receive in a traditional primary care doctor`s office setting.  We use the latest technology to ensure your condition is evaluated, diagnosed and treated – without a difficult trip to the doctor’s office.

Transitional Care

We partner with your primary Care Physician.

• We provide house calls for patients transitioning between different levels of medical care.
• Some patients have a difficult time visiting their doctors regularly. In some cases, the patient may have recently fallen, exacerbated a previously known illness, or had a surgery that makes walking a challenging task. In these situations, we coordinate care with a patient’s traditional physician’s office by helping those patients who cannot make it into the office.
• We create a partnership with the patient’s regular primary care provider. We visit the patient in their own home to avoid unnecessary re-hospitalization or exacerbation of an illness. When the patient’s health improves to where they can make it to the office, we transfer the patient back to their regular physician. Working as a partnership, NOVA House Call Physicians creates and sends a transition plan to the patient’s primary care provider which includes all visit documentation and prescription updates.
• By providing transitional care to high risk patients, NOVA House Call Physicians works to reduce the risk of re-hospitalization.
• NOVA House Call Physicians can also assist as a care provider for patients who require high level critical healthcare management for any given period of time.

Case Management & Care Coordination

By fulfilling the role of a primary care physician, NOVA House Call Physicians coordinates patient care with other health care providers.  We work diligently to decrease unnecessary visits to the emergency room and to the hospital.

With regular physician home visits, medication management, and care coordination, we work to improve the health of homebound and geriatric patients.

We work with assisted living and senior living facilities.

Many of the residents at assisted living and independent living facilities need a wide variety of physician services. At NOVA House Call Physicians, we provide these services to patients inside the facility.
